Parent Agreement, Assumption of Risk, Release of Liability, and Medical Authorization

Linas Lithuanian Heritage School of Seattle, Washington ("the School") · Version Date: 2026–2027 School Year

IMPORTANT NOTICE

This document contains important legal information regarding participation in school activities. Please read it carefully before agreeing to its terms.

1. Acknowledgment of the Nature of the School

The Linas Lithuanian Heritage School of Seattle is a community-based, nonprofit educational program dedicated to teaching Lithuanian language, culture, traditions, and heritage to children and families.

The School operates with the support of volunteer teachers, parent volunteers, nonprofit partners, community organizations, and community members. School activities may include, but are not limited to: classroom instruction; educational activities; cultural programs; performances and celebrations; arts and crafts; games and group activities; educational events; holiday events; occasional off-site activities and community events.

By registering a child to participate in the School, the parent or legal guardian acknowledges and accepts the terms outlined in this agreement.

2. Assumption of Risk

The parent or legal guardian understands that participation in educational and cultural activities involves certain inherent risks, including but not limited to: slips, falls, or minor injuries; sports or recreational injuries; injuries arising from arts and crafts activities; illness or communicable diseases; interactions with other students or participants; outdoor activities; transportation-related risks; accidental property loss or damage.

The parent/guardian acknowledges that not all risks can be prevented. By enrolling their child in the School, the parent/guardian voluntarily assumes all ordinary risks associated with participation in School activities on behalf of their child.

3. Release and Waiver of Liability

To the fullest extent permitted by the laws of the State of Washington, the parent or legal guardian agrees to release, waive, discharge, and hold harmless Linas Lithuanian Heritage School of Seattle, including its board members, officers, directors, teachers, volunteers, assistants, parent helpers, nonprofit partners, sponsors, host facilities, agents, representatives, and affiliated organizations, from any and all claims, demands, causes of action, liabilities, damages, losses, expenses, or injuries arising out of or related to the child's participation in School activities.

This release includes claims arising from ordinary negligence but does not apply to gross negligence, willful misconduct, or situations where liability cannot legally be waived under Washington law.

4. Parent/Guardian Responsibilities

Because the School is a community and volunteer-supported educational program, parents and guardians play an important role in maintaining a safe and respectful environment. Parents/guardians agree that they are responsible for: ensuring their child behaves respectfully toward teachers, volunteers, staff, and other students; providing accurate and complete registration information; informing the School of any medical conditions, allergies, medications, behavioral concerns, or special needs; remaining reachable by phone during school activities; promptly picking up their child at the end of School activities; supervising their child outside scheduled class times unless otherwise specified by the School; explaining School rules, expectations, and safety requirements to their child; cooperating with School staff and volunteers to maintain a safe and positive environment for all participants.

The School reserves the right to contact parents/guardians regarding student behavior, attendance, or safety concerns.

5. Supervision

The School provides reasonable supervision during scheduled class sessions and official School activities. Parents or guardians remain responsible for their child: before official check-in; after dismissal or pickup; outside supervised activities; during family or community events where supervision is not explicitly provided by the School.

6. Transportation

Unless specifically arranged and communicated by the School in writing, transportation to and from School activities is the sole responsibility of the parent or guardian. Any carpools or transportation arrangements between families are private agreements between parents and are not organized, supervised, or controlled by the School. The School assumes no responsibility for transportation provided by parents, guardians, or third parties.

7. Medical Authorization

In the event of illness, accident, injury, or medical emergency involving the child, the parent/guardian authorizes School staff, volunteers, or representatives to seek emergency medical treatment from licensed medical personnel, physicians, clinics, hospitals, or emergency responders as deemed reasonably necessary. The School will make reasonable efforts to contact the parent/guardian before treatment whenever possible. The parent/guardian agrees to be fully responsible for any medical expenses, hospital charges, ambulance fees, prescriptions, or related treatment costs incurred on behalf of the child.

8. Behavior and Participation

The School is committed to providing a safe, respectful, and positive educational environment for all students, volunteers, and families. If a student engages in behavior that is unsafe, disruptive, threatening, aggressive, harmful, or repeatedly inappropriate, the School may take reasonable actions including but not limited to: contacting the parent/guardian; requiring immediate pickup; limiting participation in activities; temporary suspension; removal from School activities when necessary.

The School reserves the right to make decisions reasonably necessary to protect the safety and well-being of students, volunteers, staff, and participants.

9. Indemnification

The parent/guardian agrees to be responsible for damages, losses, or claims resulting from: intentional misconduct by their child; failure to disclose relevant medical, behavioral, or safety information; violation of School rules or policies by the child or parent/guardian. The parent/guardian further agrees to indemnify and hold harmless the School from claims arising from such conduct where permitted by law.

10. Electronic Consent and Agreement

By completing registration and electronically signing this form, the parent or legal guardian acknowledges and agrees that: they have carefully read and understood this agreement; they voluntarily agree to all terms and conditions; they are the parent or legal guardian of the participating child(ren); they understand participation in School activities involves inherent risks; the information provided during registration is accurate and complete to the best of their knowledge; this agreement applies to all children listed in the registration for the current School year.

The parent/guardian further acknowledges that an electronic signature and digital acknowledgment submitted through the registration form shall have the same force and effect as a handwritten signature.

Acknowledgment

By selecting "I Agree" and signing below, I acknowledge that I have read, understood, and voluntarily agree to the terms of this Parent Agreement, Assumption of Risk, Release of Liability, and Medical Authorization. I understand that this agreement applies to all children included in this registration submission for the 2026–2027 School Year.
